NDIS Approved Quality Auditors Scheme Guidelines 2018

Purpose

This survey seeks feedback from family members, friends, carers or nominees of NDIS participants to help assess the provider’s compliance with the NDIS Practice Standards and to ensure participant rights, safety and wellbeing are protected.

Confidentiality
Responses are confidential and will only be used by Australian QC for audit assessment purposes in line with NDIS Approved Quality Auditors (AQA) guidelines 2018.
